Control	Description
Calendar Icon	Shows who and when the form was created and last modified
Square / Arrow Icon	Activate / Deactivate the form
Paper Icon	Copy control that copies the entire form design to a new form
Scissors Icon	Moves the form to a new group
"X" Icon	Deletes the form (if it has not been used to create test records)
